The Youth Specialist is responsible for undertaking intake, assessment and planning services for young carers accessing support provided by the Carer Gateway across South West Sydney, Nepean Blue Mountains & Queensland. The objective of the role is work with young carers over the phone to access information, support and referrals relevant to their caring role.
The Youth Specialist Carer Support Worker will provide carers with information on internal and external programs and services, ensuring referral pathways for carers and their families. They utilise the Carer Support Framework to undertake intake, planning and assessment and ensure that the carers meet the eligibility criteria. They assist carers in creating an action plan that reflects their goals and current support needs.
The new Carer Gateway Services have been created in the context of national reforms and represent an exciting new era for the delivery of innovative carer services.
Reporting to the Coordinator working under general direction, this role will involve:
- Support carers with a high level of customer service ensuring carers are listened to with patience and non-judgement
- Provide Intake, Planning and Assessment services to Carers as outlined in the Carer Support Framework, providing a holistic approach to carer recovery journey with a focus on capacity building
- Assisting other areas of the business when required to manage call overflows with inbound calls throughout the contact centre
- The Carer Support Planner Youth role is not limited to the listed responsibilities, and will require assistance with other areas of the program to meet the requirements of the organisation
- Work with carers to establish and implement agreed action plans within organisational delegations and in line with Carer Gateway Operations Manual
- Provide face to face, and outreach supports where identified, and/or liaise with Community Engagement teams to support face to face options
- Ensuring the service approach incorporates the following service principles: Carer Focussed, Practical and Flexible, Inclusive
- Creating an environment which Carers will find welcoming, real and inclusive
- Develop support relationships with carers which are positive and are built on Wellways values
- Working collaboratively with young carers and their families
The Carer Gateway Services consist of 6 unique service types
- Carer Support Planning: Intake needs assessment and planning
- Carer Coaching
- In-person peer support
- In-person counselling
- Carer-directed support packages
- Emergency respite care

Qualifications & Essential Requirements
- Qualifications (minimum Diploma) in a relevant field of study and minimum 2 years' experience working in community services, allied health or carer/family peer positions. Degree level qualifications highly regarded.
- Minimum 2 years experience working directly with youth
- Driver's License
- Appropriate IT skills
- Satisfactory National Police Records Check (within the last 12 months)
- Working with Children's Check (Blue Card -QLD)
- NDIS Worker Screening Check (Yellow Card -QLD)
- Right to Work within Australia
- 100 Points ID
- NDIS Code of Conduct (Free online Orientation Module)
