About the Opportunity

The successful candidate will be working with Primary Health Care Providers and their teams to build capacity and capability, improving awareness and access to safe, quality palliative care at home and support end-of-life care systems and services in the community.

Some of the tasks within this busy and varied role will include:

  • Work collaboratively with general practices to plan potential strategies to improve patient and carer access to quality palliative care services available in the home.
  • Build confidence of practice staff though practice visits and support and provision of resources.
  • Deliver education and training building workforce capacity in the area of palliative care
  • Consultation with relevant stakeholders to develop communication processes across service providers around access palliative care support and advice after-hours
  • Effectively escalate and document stakeholder issues or concerns and resolve them in a timely manner
  • Provide training and support to GPs and practice staff aimed at optimising use of systems such as My Health Record, Pen Clinical Audit Tools, and HealthPathways,
  • Ensure accurate data collection in CRM software and provide reports as required

About You

We are looking for someone who has:

  • Minimum 3 years experience in a program coordination or support role preferably in a health care or NGO environment
  • Demonstrated high level communication skills with an ability to liaise and engage with a variety of stakeholders
  • Demonstrated ability to work collaboratively, consult, negotiate and manage complex relationships
  • Intermediate computer skills including Microsoft Office Suite and experience with Customer Relations Management systems
  • Ability to work effectively independently as well as a part of a dynamic and multidisciplinary team
  • Experience working in a medical practice, or with palliative patients and their families or a demonstrated understanding of current palliative care issues will be highly regarded

To be the successful candidate you must meet the position requirements as well as provide a current National Police Check. Current NSW Drivers Licence and access to transport as regular travel is required as part of the role within the Western Sydney area is required. Full working rights to work in Australia are required for this role.

This role is a full-time contract until June 2023, with the possibility of continuation dependent on funding
