About the program

Wesley Home Care & Residential Services offers older people and those with a disability a range of services designed to enhance their wellbeing and promote independence. Care and services to support and assist older people who wish to remain living in their own homes is provided through the Commonwealth Home Support Program and Home Care Packages. These two major programs are a key focus of our service delivery in home care.

About the role

An exciting opportunity exists for an experienced professional to join our dynamic team.

The Home Care Coordinator (HCC) is responsible for coordinating entry-level home care services under the Commonwealth Home Support Program (CHSP) and Veterans’ Home Care (VHC) program. If you enjoy working with and supporting older people from a diverse range of backgrounds and life experiences, this is the position for you. Speaking with clients over the telephone, you will conduct intake and assessment and develop care plans using a reablement approach. You will conduct review of client needs and support them to navigate My Aged Care when their needs increase, making timely referrals to the Home Care Package (HCP) program. You will work with informal family carers and advocates to achieve positive outcomes for clients. You will highly value the opportunity to be working alongside clients and improving their quality of life.

Essential criteria

  • Certificate IV in Community Services, Ageing Support or equivalent and/or relevant experience in community care, home care, disability or similar setting
  • strong customer service skills with a client-centred approach and an ability to engage and build trust and rapport with clients
  • well-developed written and oral communication skills and the ability to document clearly and effectively in a timely manner
  • intake and assessment experience or relevant transferrable skills
  • demonstrated ability to focus on achieving KPI’s, with highly developed time management skills
  • collaborative team player working effectively with internal and external stakeholders
  • proficient computer skills and use of Microsoft Suite.

Desirable criteria

  • knowledge of the Aged Care Quality Standards
  • experience using CareLink+ or similar customer management systems.
