You are an emerging leader in the health or community services sector with passion for achieving health and wellbeing outcomes for people experiencing or at risk of homelessness and are ready for your next challenge.

You understand the importance of balancing client outcomes, clinical governance and developing a strong team culture all while keeping a multidisciplinary service humming along nicely. You’re well-versed in developing strategic partnerships that deliver improved referral pathways and outcomes for clients. You’ll bring a tertiary qualification (in relevant nursing, health and/or human services related disciplines) and experience in building a dynamic team that works at the front line.

You will be the captain of our After Hours Outreach team, but when we are in thick of it, you know the importance of playing off each-other’s strengths and having each-other's back – it’s the #1YP way!

You thrive on empowering teams. You know the importance of ethical and emotionally intelligent leadership practices and responsive thinking to help us provide high impact support, without judgement. You prioritise tasks based on impact and outcomes whilst supporting your team and coordinating service delivery, all while balancing lots of other plates in the air.

You’re bendy. You can switch from supporting a client to coaching a team member. You can manoeuvre your approach up, down and sideways but know the importance of boundaries and frameworks. You’re an active listener and tailor your support based on the needs of the people you are working with. Changing lives is your passion but you still embrace rolling up your sleeves and mucking in to get the job done.

You see the person, You think outside the box for ways to meet clients and team members where they're at and believe that engagement shouldn’t be a one size fits all model. You always see the person, not just their immediate situation, because you understand that everyone has different needs - and that is okay.

A bit about the role:

Our After-Hours Outreach Service (AHOS) team utilises an assertive outreach model to assist adults with complex health needs who are primarily vulnerable, sleeping rough, living in insecure accommodation or accessing homelessness services in Melbourne.

The After Hours Outreach Service Coordinator will be based out of The Living Room on Melbourne’s iconic Hosier Lane and is a part-time role that requires 36 hours each week. Those hours will be a mix of day and night hours to allow the right person to form strong working relationships with different services operating day and night.

If you worked here this past month here are some things you might have been involved in:

  • Coordinating outreach and service coordination to clients who are, or at-risk of, experiencing homelessness.
  • Overseeing best practice in opportunistic health and wellbeing interventions, joint care planning and discharge planning.
  • Completing outcome and impact reporting for your program.
  • Actively building networks and attending external meetings to create strong strategic partnerships to improve system access and coordination.
  • Participating in assertive clinical outreach to engage and assess adults sleeping rough in Melbourne’s CBD utilising appropriate interventions.
  • Leading a multi-disciplinary team of community and mental health nurses and outreach staff to ensure high clinical standards.
  • Supporting our After Hours Outreach team with problem-solving, coaching and building their skillsets.
  • Working with fellow emerging leaders on organisation-wide improvement projects.
  • Developing practice partnerships with outreach services, emergency departments and other homelessness services.
  • Improving health and housing outcomes for people sleeping rough in Melbourne’s CBD.

A bit about us:

The Living Room is a primary health service, the team works with people aged 18+ to provide free healthcare and support to improve the physical, mental and social well-being of individuals who are, or at risk of experiencing homelessness.

The only service of its kind in Melbourne, focuses on the social determinants of health to look beyond disease or illness, to see a person's overall situation. Our multi-disciplinary team works with individuals to identify goals and focus on harm minimisation, self-determination, and empowerment.
