Youth Support + Advocacy Service (YSAS) is one of Australia’s largest and most comprehensive, youth specific community service organisation that enables young people experiencing serious disadvantage to access the resources and support they require to lead healthy and fulfilling lives.YSAS is a Child Safe organisation. This service is supported by funding from South Eastern Melbourne Primary Health Network through the Australian Government’s PHN Program. The BounceBack Program is commissioned through the SEMPHN, and supports young people (12-25 years) with severe mental illness who have complex needs and will benefit from a flexible and responsive service offering therapy, case management and coordination of their care over an 6 – 9 month period of time. The program is colocated and integrated within a community of youth services as a part of the youth mental health stepped model of care at the headspace Frankston site.
This role will ensure that the care delivered to young people is of the highest possible standard and that young people and their families/carers receive timely, responsive and appropriate services to meet their presenting and emerging needs. The role provides increased capacity to engage and support young people with complex needs and works closely with internal stakeholders such as GP's, psychiatrists, centre-based intake clinicians and allied health practitioners along with other colocated or regional multi-sectorial youth service providers to facilitate coordinated care and treatment for moderate and complex mental health disorders. This role will conduct assessments, provide evidence based mental health interventions and psychosocial interventions, case management, family work, coordination of multidisciplinary care, develop service linkages, and provide co-delivery of groups and services with other clinicians.
