360 Health Management provides high-quality support services and individualised strategies to clients of all ages and abilities within the NDIS. Due to growth, we are seeking a Behavioural Practitioner who demonstrates initiative and a strong desire to achieve outstanding outcomes for both their team and clients. It is essential to have experience working with complex clients from a person-centred approach.

This role can be based in our Shepparton or Echuca office, with travel required at times between the two offices. We would consider part time for the right person.

Goals of the position

  • Manage your own caseload of participants, primarily developing person-centred strategies for individuals with complex or comorbid disabilities.
  • Mentor and train a small team of Behaviour Support Practitioners.
  • Monitor and approve Positive Behaviour Support Plans.
  • Create, supervise, and ensure behaviour support plans meet clinical and NDIS requirements.
  • Prepare monthly reports to the NDIS where required.
  • Develop, monitor, and action individualised risk plans to mitigate incidents and hazards.

About you:

  • A degree in social work, psychology, or developmental education.
  • Must have an empathetic disposition, with the ability to liaise with internal and external stakeholders.
  • Experience in providing clinical and/or behavioural assessment, intervention, and support.
  • Knowledge of the NDIS, and relevant disability legislation.
  • Must have a valid driver’s licence.
  • Must have an NDIS Worker Screening Check or be willing to obtain one.
