Our client is looking for an NDIS Case Manager to work in their Melbourne office with the right attitude, knowledge, experience, and skills to be part of their team that drives strategy implementation across all business operations.

Position objectives

As an NDIS Case Manager, you will assist NDIS participants in activating, interpreting and implementing their NDIS Plans. You will do this by identifying services and supports that meet participants' needs, maximise participants' support, and enable participants to control their support. You will coordinate service providers and supports, liaise with government agencies, obtain quotes for services, organise service agreements, monitor participants' plans and develop goals before the second plan and support required to achieve them.

Key Performance Indicators

  • Documented evidence of progress against an individual’s service and support schedule.
  • There is clear evidence of person-centred and individualised approaches being adopted.
  • Uses survey and feedback mechanisms to assess service standards and quality benchmarks.
  • Achieves required standards and quality benchmarks for participants and participant services.
  • Ensures complaints are addressed according to the organisation’s policy and procedure, including escalating appropriately when required.
  • Reports regularly on appropriate and agreed on participant interaction and related activities.

Tasks and duties

Undertake comprehensive participant assessment and planning by assessing, monitoring and reviewing the participant's needs.

  • Develop and manage participant service schedules, including service planning, confirmation, short-notice placements, and cancellations.
  • Manage day-to-day activities of the service.
  • Monitor participant budgets in accordance with program guidelines.
  • Manage communication with key stakeholders - both internally and externally.
  • Liaison with roster coordinators, case advisers/managers, accounts and any relevant external parties
  • Receiving referrals and assessment of participant support needs.
  • Develop an understanding of participant needs to inform the interpretation of their NDIS support plan.
  • Research, assess and source several mainstream, community, informal and provider options.
  • Present preferred options or providers to participant and/or plan Nominee.
  • Negotiate services and prices for any quotable supports.
  • Organise assessments, as required, to determine the nature and type of funding required (e.g., Assessment to determine the type of complex home modifications required).
  • Calculate the budget for each support type and advise any relevant plan manager of the breakdown of funds.
  • Liaise with any plan manager to establish the appropriate claim categories and attribute the correct amount of funds.
  • Assist the participant in preparing for their plan review by working collaboratively to assess the achievement of their goals and if they believe they obtained value for money for their Plan.

Qualifications / Other Requirements

  • Case Management, health, NDIS support or business-related qualifications plus extensive experience in community services or business management fields.
  • Extensive experience in providing high-quality support to people with disabilities.
  • Unrestricted Driver’s licence.
  • Be an Australian citizen or permanent resident.
  • Current First Aid certificate.
  • Working with children check.
  • National police check
  • Pre-employment medical and fitness report.
  • Qualification in workplace training and assessment is desirable.
