The Role

  • The Team Leader – 1:1 In-Home is responsible for the co-ordination and leadership of the 1:1 In-Home program providing support to our participants in their own home and other areas of their daily routines.
  • Working cooperatively as a member of the Blossom Support Services Leadership and admin team the Team Leader will:
  • Manage and mentor staff in the delivery of person-centred activities supports to participants;
  • Review individual plans and other documentation in consultation with key stakeholders;
  • Lead the management of participant behaviours and ensure strategies are in place to minimise risk;
  • Coordinate and manage the daily operations of the program including staff supervision, development, management, and rostering;
  • Work 2 hours of administrative work each day;
  • Provide approximately 4 hours of active support for 5 days each week (Approximately = 30 hours per week);
  • Be required weekends, mornings and afternoons and after hours as necessary;
  • Central Office located at Torrensville.

What you need to bring

Essential:

  • Strong communication skills and the ability to liaise with a wide group of stakeholders to achieve positive outcomes;
  • High level organisational skills;
  • Valid Department of Human Services (DHS) Working with Children Check;
  • Valid NDIS Worker Screening Check;
  • Valid First Aid Certificate & CPR;
  • Valid Australian Driver's Licence and willing to use personal vehicle;
  • Willingness and availability to work evenings and weekends;
  • Ability to comply with COVID-19 vaccination requirements;
  • Ability to participate in the out-of-hours on-call roster, on-call allowances will apply as per the award;
  • Demonstrated experience supporting clients with complex needs and behaviours of concern.

Desirable:

  • Experience managing, leading and supervising staff;
  • Experience developing plans, programs and reports relating to client supports.
