We are currently seeking applications for a Physiotherapist to join our Kingsley Transition Care Team on a permanent full-time basis working 76 hours per fortnight. The Brightwater Transition Care Programme is a 12 week, time limited and goal directed program aimed to optimise clients’ health, wellbeing, and independence for a smooth transition from sub-acute care to living at home or transitioning to aged care.
The Transition Care Physiotherapist will be responsible for completing initial physiotherapy assessments to understand each client’s unique goals and contribute to the development of each clients individual care plan. The Physiotherapist will also be responsible for developing an individualised therapy program for each client to support them to reach their rehabilitation goals. The Transition Care Physiotherapist will be actively involved in providing rehabilitation services to the 41 clients in Transition Care, these rehabilitation services cover a wide range of different diagnoses including stroke, Parkinson’s and orthopaedics. The Physiotherapist role includes supervision of a team of therapy assistants in collaboration with the site Occupational Therapist. The Physiotherapist plays an active role in the weekly site MDT to support safe and effective discharge planning.
