CCNB is one of the largest community-led organisations in the Northern Sydney Region, providing services to older people, those with dementia, people with disabilities, severe and persistent mental illness as well as those with complex health needs. We work with people, their families and communities. CCNB is seeking a dynamic and experienced person to assist people to successfully navigate the care and support system and to enhance their ability to access the support/s they require.

The role

Reporting to the Manager, Community Services, the Care Coordinator will understand the specific needs of the client and work with them and their own network to access the supports they require. The Care Coordinator will also work with relevant others in the community to ensure the desired and necessary care and support is provided to the client and advocate for systemic changes to occur when a clear and persistent issue arises. A CCNB Care Coordinator provides impartial information advice and guidance to people to source appropriate providers as well as information and mechanisms to access care and support more independently.

About you

• Qualifications in Social Work, Aged Care, Occupational Therapy, Allied Health Professional or equivalent
• Experience working in community care organisations
• Commitment to person first and person centred approaches to care
• Experience in aged care / NDIS and/or health services
• A passion for working with people to ensure they get the most out of life
• Current driver’s license
• Second language desirable (particularly Mandarin, Cantonese, Korean ,or Italian)
