About the Organisation
CCNB is one of the largest community-led organisations in the Northern Sydney Region, providing independent and impartial information, advice and guidance services to older people, those with dementia, people with disabilities, severe and persistent mental illness as well as those with complex health needs. We work with people, their families and communities.
CCNB is seeking a skilled and experienced social worker to provide information, advice, and guidance to the Care Coordination Team and will liaise and coordinate allied health services for a wide range of client groups. The social worker will also provide clinical reports for the person navigating health care systems to enhance their ability to access the support(s) they require.
About the Role
- Provide knowledge and expertise as a social worker to support clients in CCNB services
- Liaise with external allied health service providers to conduct home based OT assessments including all areas of activities of daily living and ensure scope of work is completed.
- Communicate assessed needs clearly and precisely to client and funding organisations within appropriate time frames
- Design and deliver client and staff capacity building information, advice and guidance sessions.
- Develop and maintain engagement with a variety of stakeholders to ensure CCNB is well connected with a variety of support providers in the community and to build capacity within the sector.

About You
- Current AASW social worker registration
- NSW Driver’s Licence, Class C
- Understanding and commitment to a wellness approach to service delivery
- Demonstrated knowledge and experience in mental health, aged care, disability, chronic health care.
- Demonstrated knowledge and experience in providing safe and effective clinical care tailored to the clients’ needs, goals and preferences to optimise their health and well being
- Demonstrated knowledge and experience in providing safe clinical care tailored to the client’s needs and goals
- Understanding of contemporary treatment, support and community development approaches for people with high and complex are needs living in the community.
- Detailed understanding and experience of relevant national and state legislation and health and community care services systems and standards.
- High level oral and written communication, consultation, research, report writing and networking skills.
- Demonstrated ability to operate independently and professionally with limited supervision.
- High level of proficiency in using technology-based computer programs (CRM’s, Microsoft office suite)
- Triple vaccinated for Covid-19 (by public health order)
- Current NDIS worker screen & Working with Children Check (or the ability to obtain one).
