Description:

We are currently seeking an Aboriginal Health Worker to join our team to provide support for individual community members and the community to maintain and promote health care. 

The Aboriginal Health Worker will work as a member of the health team to provide a range of health services that assist in prevention, early detection and management of health and underlying social issues.

This role requires a flexible, holistic, and sensitive cultural approach to supporting community members to access services.

Key Selection Criteria:

  • Be an Aboriginal person as recognised within their Aboriginal community.
  • Certificate III or IV, Aboriginal and Torres Strait Islander Health
  • A demonstrated knowledge of Aboriginal communities and cultures and an understanding of the issues, particularly in the health area which are affecting Aboriginal people and a demonstrated ability to communicate sensitively and effectively with Aboriginal people.
  • Demonstrated knowledge in the principles of primary health and associated skills to perform assessments, plan and implement health care strategies.
  • Ability to develop and run health information sessions and health promotion activities
  • Demonstrated interpersonal skills in dealing with, community patients and staff members.
  • Demonstrated knowledge and experience in using computers and particularly with Microsoft Suite and a demonstrated understanding of medical software.

  • Provide culturally appropriate health care to meet the needs of the community.
  • Participant in relevant local community projects and groups that aim to increase general awareness to promote positive health practises for all clients
  • Provide clinical outreach services including screening and review, home visits and hospital outreach, follow up care for patients discharged from hospital.
  • Develop and deliver health education to clients
  • Assist in case management support to individuals and their families.
  • Provide information about available services and assist clients to access these services through appropriate referral.
