About the Role
The Care Planner and Coach is responsible for the provision of high-quality client services to support and develop the capacity of carers living in the ACT. The Care Planner will assess carer’s needs, develop support plans and budgets and coordinate support services for carers. The position liaises with brokerage services and other service providers. Effective February 2021, the care planner will also provide coaching to carers under the Carer Gateway Coaching model.
This role reports to the Team Manager -Carer Support Services
Duties include but are not limited to:
- Using the Carers Star™ undertake a comprehensive and holistic needs assessment considering all relevant aspects of the carer’s circumstances
- Identify what types of supports/services will benefit the carer, in line with their assessed needs
- Identify relevant support and services
- Develop an Action plan to document the carer’s goals and the actions that will be undertaken to support them
- Complete the Action Plan by recording the services the carer has chosen to use and action any referral
- Planning supports for a carer must accommodate needs and preferences for service delivery that are specific to their cohort (including but not limited to Aboriginal and Torres Strait Islander, Culturally and Linguistically Diverse (CALD) and Lesbian, Gay, Transgender, Intersex and Queer (LGBTIQ) people).
- Where the carer is eligible for financial support package, develop a budget and expenditure plan in accordance with guidelines.
The job will be full time, with hours to be worked between 9 am to 5.00 pm Monday to Friday.
Successful applicants must have:
- Qualifications in community services or health, aged care, mental health or disability fields
- Experience and/or qualifications in the delivery of care planning, case management or intake and assessment processes;
- Demonstrated empathetic and flexible approach to manage the sensitivities of carer issues
- Excellent computer skills including proficiency in Microsoft Office suite and use of databases
- Highly developed client documentation, record keeping and file management skills with an attention to detail
- Ability to build and nurture relationships and networks with service providers and target groups and within the ACT community
- Highly developed organisational, communication and interpersonal skills (both as part of a team as well as individually) and competency in liaising with a wide range of stakeholders
- Demonstrated understanding of caring issues within a community environment to coordinate quality service delivery
- Demonstrated understanding of privacy legislation and ability to work within these parameters
- Current National Police Record Check
- Current Working with Vulnerable People Registration
- Current Driver’s License (essential to undertake client meetings at locations other than the Holt office).
