The Cassidy Crew is a fast growing innovative For Purpose Social Enterprise working in the ever-changing field of NDIS.
We are constantly expanding and require a bright, experienced, participant focused Support Coordinator to be based out of our Warragul office. The position will be full time however can be flexible and/or part time for the right candidate.
As a Support Coordinator you will provide varying levels of coordination and transition supports for children, young people and adults.
Support Coordinator’s primary focus is to:
- assist the Participant to develop knowledge, experience and connections with the community and broader systems of support.
- work together with participant to understand Plan funding and its purpose. Support Coordinators will understand the Participant’s confidence and skills, and help participants identify what they want from services. Will develop and design support solutions to meet Participant outcomes.
- assist the Participant to identify and consider support options, and link the Participant to the broader systems of supports. Where practical create a supports and action plans to facilitate the participants to implement their plan.
- assist to resolve points of crisis and develop capacity and resilience in the Participant's network.
- coach the Participants through challenges that come up. Help Participants prepare for review and report on achieved Participant outcomes
KEY RESPONSIBILITIES AND DUTIES FOR THIS POSITION:
- Manage your own time, set priorities, plan and organise your own quality work to achieve specific objectives.
- High level knowledge and skills in coordination of supports to assist people with a disability in their community or in their home.
- Excellent Understanding of the National Disability Insurance Scheme Act 2013 and understanding of the policies and procedures generally associated with the provision of Disability Support Services.
- Identify potential crisis situations and proactively manage and coordinate with minimal support from manager.
- Understand the different levels of Support Coordination with each individual you support (Support Connection, Coordination of Supports, Plan Management and Life/Transition Planning).
- Work within the funding levels associated with each client (continually track and monitor billable hours)
- Takes responsibility for own work outcomes and assists others to understand role and responsibilities.
- Continually gather and document relevant information about clients from other services for the development of evidence-based reports and updates.
- Collaborate with other service providers to achieve desired outcomes for the participant
Qualifications and Experience
Relevant Qualifications:
Relevant degree with relevant experience, or associate diploma with substantial experience, or qualifications in more than one discipline or less formal qualifications with specialised skills sufficient to perform at this level, or attained through previous appointments to undertake the range of duties required.
COVID vaccinations, NDIS worker screening check, Working With Children's Check and other online trainings as directed on employment.
