The Team:

The Support Coordination Team at Connected Self is funded through the National Disability Insurance Scheme (NDIS) which is a federal government funded scheme for people living with a disability, their families, and carers. Our professional, qualified, and experienced team provide Coordination of Supports (level 2) and Specialist Support Coordination (level 3) for children, young people, and adults primarily with a diagnosis of ASD, intellectual & psycho-social disabilities.

Connected Self Specialist /Support Coordinators are specialised in working with young people and children with high and complex needs, working closely with statutory mainstream services including the Department for Child Protection, Exceptional Needs Unit and Youth Justice.

The Role:

The NDIS Specialist/ Support Coordinator will be responsible for establishing a positive collaborative relationship with the NDIS participant, and their support network, and assist the person to identify, link with and coordinate informal, community, mainstream, and formal supports. You will help participants build their capacity by assisting them to understand their NDIS plan, build skills, overcome barriers, and achieve their stated goals.

The NDIS Specialist/ Support Coordinator will need to have a strong understanding of the NDIS and experience in the role of an NDIS Support Coordinator or similar. The role involves both outreach and office-based administrative duties, including report writing. Strong networking skills and an ability to build rapport with a wide range of people will be vital to the success of this role.

There are more than one role available on a part and full-time basis (minimum 0.6FTE), to be negotiated.

Key Responsibilities:

  • Engaging with participants and collaboratively implement NDIS plans.
  • Build collaborative relationships with participants, families, stakeholders, and service providers.
  • Strengthen and enhance participant’s abilities to self-direct their own supports.
  • Maintain case notes, produce progress reports and other written materials as required.
  • Review participants budgets and reporting.
  • Management of multiple/complex barriers and access appropriate supports with multiple stakeholders.
  • Work as part of a skilled team of Support Coordinators as well as autonomously in the community.

Key Requirements:

  • Experience working in a NDIS Support Coordination role or similar.
  • Minimum Certificate IV in Disability, Individual Support or similar.
  • Bachelor of Psychology, Social Work, Developmental Education, Allied Health or similar (desired).
  • Strong understanding of the NDIS and support planning.
  • Experience supporting people with disabilities as well as working with people from diverse backgrounds, values systems, and cultural beliefs.
  • High level interpersonal communication skills (listening, verbal, written) as well as proficient in Microsoft Office.
  • High level organisational skills including time and budget management.
  • Good knowledge of local services providers, both mainstream and formal NDIS supports.
  • Must hold and maintain a current and valid unrestricted South Australian Driver’s license and have a well-maintained, insured vehicle.
  • Must hold a current NDIS Worker Screening Check & DHS child-related employment check.
  • Must have full COVID vaccination, or have a booking, as mandated by the South Australian Government.

Classification:

The SCHADS Level will be determined based on the qualifications, skills, and experience that you bring to the role.
