Eastern Palliative Care Association Incorporated, is a not-for-profit organisation that provides home-based specialist palliative care to clients and their carers living within the Eastern Metropolitan Region of Melbourne.

This role services Priority Response clients and also services clients in Aged Care Facilities. The focus of the Priority Response part of the role is the provision of prompt and effective psychosocial assessment, support and spiritual care to clients with specialist palliative care needs and their families. Priority Assessment occurs within four hours of acceptance of referral (within office hours). This role provides a prompt response to clients who live in their own home and within aged care and disability care homes in our region, providing advice and support in relation to clients with specialist palliative care needs. The successful candidate will manage competing priorities with ease, work closely with our Nurse Practitioners and utilise their strong clinical knowledge to achieve excellent client outcomes.
