EPIC specialises in NDIS Mental Health Support Services and we are passionate and dedicated to working with vulnerable people and making sure that they don’t slip through the cracks.

We are currently seeking an experienced NDIS Support Coordinator in the Northern to South Eastern Melbourne suburbs.

About the role:

The Support Coordinator assists NDIS participants to build the skills they need to understand, implement and use their NDIS plan. The Support Coordinator will work with participants to engage with supports to increase their capacity to maintain relationships, manage service delivery tasks, live more independently and be included their community. Support Coordinators also assist participants who have high or complex needs to help manage challenges in their support environment which may include health, education, or justice services. The support coordinator aims to reduce barriers to participants implementing or using their NDIS plan.

Qualifications & experience

  • Must have experience or extensive knowledge/training in NDIS support coordination
  • Proven experience working with people with mental health conditions
  • Candidates with extensive industry experience in crisis and specialist work will be highly regarded
  • Degree qualified in Social Work, Psychology or relevant to the field of human services or allied health. Strong NDIS experience along with Cert IV or Diploma qualifications will also be considered.
  • Experience/Certificate AOD will be highly regarded

Tasks & responsibilities

  • Implement NDIS plans in a timely manner
  • Provide intensive person centred support for participants
  • Develop support plans and complete support coordination progress reports
  • Respond to and support participants experiencing crisis
  • Networking and collaborating with community and government agencies
  • Ability to liaise with care teams, informal and formal supports and legal services
  • Well-developed interpersonal and communication skills, both written and verbal
  • Strong organisational and administrative skills

Other requirements:

  • Current Police and Working with Children Check
  • First Aid/CPR certificate
  • 2 professional reference checks
  • Current full driver's license and own vehicle
  • Commitment to providing exceptional support together with great communication skills

All staff must complete an NDIS worker screening check prior to commencement in the role.
