Fronditha Care is a longstanding Aged Care Provider committed to delivering services to Australian elders of Culturally and Linguistically Diverse Backgrounds. We focus on providing unique services both in residential and community care for elderly people of Greek speaking background in Victoria and of multilingual background in New South Wales.

An opportunity has arisen for a highly motivated and experienced individual to join our Community Services Division as a Home Care Packages (HCP) Advisor, located at the Support Hub in Mulgrave.

This is a Full Time, Fixed Term 12 month position. The successful candidate will be a lateral thinker who thrives on ‘looking outside the square’.

About the role

The position of a HCP Advisor is responsible for the following:

• Work within Fronditha Care’s Consumer Directed Care Model of Service.
• Ensure service delivery meet all associated funding agreement, legal standards and the Aged Care Act 1997 as well as Fronditha Care’s quality assurance goals.
• Undertake appropriate assessments and care plans for the clients whilst maintaining and ensuring clients’ records are up to date.
• Monitor records of clients’ budget plans.
• Be on after hours ‘on call’ service on a rotating basis within the Home Care Packages team.
• Have a ‘can-do’ attitude
• Provide leadership and management to direct care staff through proper support, supervision and training.

The successful candidate will have qualifications in social sciences, work experience in the field, as well as an understanding of the ageing process and dementia care.

• Leadership skills.
• High level of motivation and problem solving skills.
• Advanced communication and negotiation skills.
• Ability to speak Greek is highly desirable as well as a current driver’s license with access to own vehicle.
