About you:

We are seeking a passionate, committed, highly motivated person who has experience in working with homeless and/or marginalised young people through a responsive centre based and mobile support service to connect with the young person in their community. 

The position will provide intensive case management support to assist young people to access or maintain housing, education and employment opportunities. 

Target Group:

• Young people 12 – 21  years at risk of disconnection from family, education and/or employment and at risk of experiencing homelessness;
• Young people 12 – 21 years currently at risk of, or involved with the Child Safety, Youth Justice and/or Legal Systems 

Main Responsibilities:

• Engage with the target group through Centre Based Support and Mobile Support;
• Assess needs, develop and implement case management plans for individual clients and provide ongoing support;
• Specific focus for the role on assisting young people to avoid disconnection from family, education and community networks;
• Assist young people to develop life skills including education and employment pathways with the objective of moving towards a healthier, safer and positive lifestyle;
• To establish key relationships in the youth sector/community; with government services and community organisations to facilitate appropriate referral pathways;
• To represent Gold Coast Youth Service within the region;
