About Community Interlink :

Community Interlink is a consortium model consisting of 18 local Public Health services. Community Interlink provides Aged Care Case Management and Support Coordination to NDIS recipients. Community Interlink pride ourselves on supporting local people by local people. This role is located in Shepparton and is supported by the Shepparton team. The servicing region will cover Shepparton and surrounds supported by the Shepparton Program Manager. About the department:

Community Interlink is an equal opportunity employer and consists of about 50 staff throughout our the Hume and surrounding Regions. These staff are situated in various locations across the servicing area. This role will be predominately servicing Shepparton and surrounds consumers to utilise their Home Care Package funds to maintain independence and wellbeing and NDIS participants to understand and utilise their NDIS plans. The Community Interlink team is a supportive and inclusive environment that will support your learning and development.

About the role

This is a dual role managing both Home Care Package and NDIS recipients. The successful candidate will be supported to learn and grow in both areas and potentially specialise in an area of your choosing. You will support Home Care Package recipients to understand their funding and source supports to meet their individual needs. You will help them to maintain their independence and safety in their homes for as long as possible by mitigating risk and support ageing well. As a Support Coordinator you will be the interpreter between your participants and the NDIS. You will learn how to read and interpret their plan and goals and help them to engage and monitor services to meet their needs.

You will have access to to a team of highly skilled and supportive Case Managers and Support Coordinators to ease you into this role and guide you through it.
