Gateway Health are excited to be launching the new CP@ (Community Paramedic) program within Gateway Health. This is an innovative, place based approach to supporting community members who are disconnected from their community and the healthcare system.
The program will primarily focus on underserviced rural/regional areas and with the people who need the service the most. The CP@ program is a chronic disease prevention, management, and health promotion service. The aim of the CP@ is to support members of the community manage their health outside of the emergency setting.
Internationally, the program has demonstrated a significant reduction in emergency call outs, improved management of chronic conditions, and improved empowerment and social connectedness for people engaged with this service.

We are currently seeking highly skilled Paramedics who have a strong clinical background and experience working within the community health industry.
