Home Caring Footscray is an NDIS registered and Aged Care Provider located in Footscray, VIC. We are dedicated to serving clients with diverse needs ranging from core support, accommodation services and specialist support coordination. Our team works with participants, families, allied health professionals, plan managers, and the NDIS to ensure our clients get the services they need. We are committed to serving all participants in our community, including those with complex needs and those with a forensic background.

Role Description

This position is a key member of the NDIS Services Team providing Support Coordination for eligible clients. The Support Coordinator is responsible for supporting clients in their NDIS journey by ensuring participants funds are being spent wisely and that they are connected with appropriate services as part of their NDIS plan. They will assist in managing support budgets, service agreements, and billing compliance. They will work collaboratively with other support providers and external stakeholders to ensure the best outcomes for clients. Applicants with native-level proficiency in Vietnamese would be highly desirable to support our Vietnamese speaking clients.

Qualification and Compliance:

  • Relevant experience in NDIS support coordination and implementation or case management experience supporting clients with high complex needs, successful completion or working towards a Cert IV/ diploma/degree/post grad in Social Work, Social Welfare, Psychology or related discipline
  • National NDIS Worker Screen Check or willing to get one
  • Victorian Working with Children Check
  • Australian driver's license and reliable vehicle
  • Worker Rights in Australia
  • Proficiency in Vietnamese would be desirable

Main responsibilities and requirements:

Person centred assessment and care planning

  • Possess sound knowledge of the NDIS Act 2013, NDIS Rules 2013, and the National Disability Insurance Scheme Operational Guidelines 2013 and be able to apply same
  • In consultation with clients, carers and families facilitate service planning and review
  • Develop and monitor NDIS Service Agreements and plan budgets with regular reviews/case conferencing
  • Manage any identified risks within principles of Positive Approach to Challenging Behaviours and Duty of Care, Dignity of Risk
  • Work within legislated NDIS frameworks

Client and stakeholder engagement

  • Build positive relationships with clients, families, and external stakeholders
  • Make relevant referrals, broker services and deal with suppliers as per NDIS plan
  • Develop and maintain relationships with community organisations to ensure a holistic approach, free from conflict of interest
  • Develop and maintain relationships with NDIS representatives to ensure high quality communication and service is provided
  • Ongoing communication with clients, carers/families to ensure they are aware of rights and responsibilities to make informed choices.

Organisational Responsibility

  • Be aware of and comply with legislation and organisational requirements relating to Workplace Health and Safety
  • Contribute to quality processes such as management of client feedback
  • Actively participate in team meetings
  • Actively participate in professional development activities (internally and externally) to maintain contemporary knowledge and skills as well as acquire new knowledge and skills

Remuneration commensurate with Experience and Qualifications. Open to discussion. The latest COVIDSafe Practices are in place. This is a hybrid role. Office is based in Footscray.
