About the Role
A fantastic opportunity for a Case Manager, Assertive Outreach to join the Homelessness team based in Berri, SA on a full time basis until 30/6/2018.
 
Our Riverland Homelessness program, is an assertive outreach and intensive case management support service, designed to engage with individuals experiencing homelessness. The team has a commitment to ending the cycle of homelessness and improving the health, mental health and social well-being of individuals through promoting the recovery model.
 
The program recognises that individuals experiencing homelessness often have other contributing factors and complexity of needs. Therefore the role involves completing in-take and assessments, case planning, case work, case management and active referrals to support people in a holistic manner to address the complexity of issues that may be impacting on their well-being to better support them into housing options.

Skills & Experience
  • Minimum Certificate IV in Community Services or tertiary level qualification in Human Services or Equivalent.
  • Demonstrated experience in the Homelessness, mental health or other health and welfare related fields with significant co-ordination experience.
  • High level communication, influencing, liaison and negotiation skills including sound written and verbal skills.
  • Ability to develop positive relationships with a broad range of internal and external stakeholders.
  • Strong written and computer skills for the creation of plans, programs, reports, correspondence and other documents.
  • Established problem solving and conflict management skills.
  • Commitment to the values of dignity, respect, inclusion empowerment, confidentiality and strength-based practice.
  • Current Driver Licence.
  • Current First Aid certificate.
Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
