About the opportunity
Working across North Western Melbourne and Eastern Melbourne PHNs, the Manager will be responsible for planning, delivery, monitoring and evaluating the HealthPathways Melbourne program. The program supports the delivery of best practice, evidence-based quality and safe care for patients through:
- Mobilising partners across the region to a common goal through collaboration
- Developing gold standard clinical and referral pathways
- Working together as a collective team
- Living our PHN values
The result is a free website with relevant evidence-based information on the assessment and management of a range clinical conditions, including local referral guidance that is designed to be used by health professionals at the point of care.
A typical day for the Manager may involve:
- Developing and maintaining customer-service orientated relationships with local leaders, from clinicians to community, government, professional and peak bodies to support patient outcomes
- Leading and coaching your team, building their capacity and mentoring them through change management initiatives, supporting a positive, continuous improvement culture
- Working with the HealthPathways Governance and Reporting Committee to progress strategic priorities using data-driven, stakeholder informed insights and information
- Liaising with your manager, providing updates and reports on the progress of operational and team plans

We are looking for someone who can:
- Lead and manage a team of remote and on-site staff in a dynamic and changing environment
- Coordinate and effectively manage programs within a complex governance arrangement, including an ability to work effectively under pressure
- Build effective relationships and collaborative relationships with leaders in our community to deliver outcomes
- Communicate with positive influence, approaching decisions, actions and perceptions with a convincing and balanced rationale
The successful candidate will be required to undertake a National Police Check.
