The Program Officer, HealthPathways will work with GP clinical editors and other key stakeholders to support the development, implementation and monitoring of the HealthPathways Melbourne program. This includes managing HealthPathways content development and organising Clinical Working Groups.

HealthPathways is a collaborative and structured approach to coordinating patient care across acute and primary care settings. It brings together general practitioners, specialists, nurses and allied health professionals to discuss optimal assessment and management of common medical conditions, including when and where to refer patients.

What you'll need to be successful in the role:

  • Strong experience in program and/or project management and stakeholder management
  • Tertiary level qualifications
  • Relevant qualifications in public health, health promotion, health service management, public administration, clinical studies, or a related field
  • Experience working with general practice, community health or local hospital networks
  • Excellent writing skills with the ability to prepare complex briefs and reports using clear, concise and grammatically correct language

We are North Western Melbourne Primary Health Network (NWMPHN), Victoria's largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in our community.

PHNs have been established with the key objectives of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and improving coordination of care to ensure patients receive the right care in the right place, at the right time.
