Your new role

You will be working as part of an established multidisciplinary team to provide direct and comprehensive high quality and evidence based occupational therapy services to clients who are aged under 65 (under 50 for Aboriginal and Torres Strait Islander People). This role is funded by Home and Community Care, Program for Younger People. The model of care aims to maximise clients’ and carers’ functional independence and their ability to participate fully in the community.

As the Grade 2 Occupational Therapist in the Prevention of Chronic Illness Care Team you will be responsible for:

  • Providing high quality, evidence based occupational therapy with clients aged under 65
  • Occupational Therapy service provision in client’s homes, community settings and/or at Merri Health sites
  • Health education, on both a group and an individual basis, with a particular focus on chronic conditions and restoration of function (for example in our Chronic Pain Service)
  • Working with a diverse caseload of clients under the HACC-Program for Younger People
  • Potential to develop supervisory skills through student placements
  • Opportunities to participate in service development initiatives
  • Note: This is not an NDIS funded position; however may involve supporting HACC-PYP clients with their NDIS access

What you’ll need to succeed

To be successful with Merri you will go above and beyond to achieve great results for our clients, the organisation and yourself. We’re looking for individuals with:

  • Degree in Occupational Therapy and current registration with AHPRA
  • Minimum 2 years experience providing Occupational Therapy assessment and management with adults and younger clients with functional impairments related to disability, chronic conditions and/or short term health conditions
  • Proven ability to work autonomously and in a multidisciplinary team
  • Experience completing autonomous home and community based assessments and interventions with clients
  • Experience in the application of the principles of Chronic Disease Management; Person Centred Care; Goal Directed Care Planning and Self-management
