About the role

The Mental Health Integrated Complex Care (MHICC) service is a key element of the SEMPHN Mental Health Stepped Care Model. MHICC supports consumers with severe mental illness who have complex needs and who will benefit from improved coordination of care and cannot access the NDIS. The service consists of clinical nursing services, family support and liaison, care coordination and liaison (clinical and non-clinical services), and improving access to psychiatric and psychological care.

We are seeking a driven, compassionate permanent, full time Lead Practitioner - Care Coordination & Liaison to take on this challenging opportunity in the Dandenong region. As the Lead Practitioner you will mentor and engage a close knit team to provide high quality, safe recovery orientated care to clients, families and carers to enable better quality of life.

Key Responsibilities

  • Coach and mentor the team to deliver high quality services and facilitate positive educational and social experiences.
  • Purposefully engage with clients and their service supports using clinical techniques.
  • Support families, existing support networks and stakeholders to assist clients through treatment using family inclusive interventions.
  • Work collaboratively to ensure a coordinated and integrated response to the client’s recovery goals.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with agencies, clinical providers and key stakeholders.
  • Proven ability to effectively lead and develop a team to achieve quality service outcomes and experience in the provision of professional supervision.
