About the Company

Mind Australia Limited is a leading community managed specialist mental health service provider. We are also now a registered NDIS provider. We have been supporting people dealing with the day-to-day impacts of mental illness, as well as their families, friends and carers for 40 years. Currently we have over 50 service sites throughout Australia that, this year, will provide about 19,000 service engagements to people on their personal recovery journeys.

About the role

Support consumers to achieve sustainable mental health care. As a Support Facilitator, you will join the Partners in Recovery (PIR) team based in Nunawading. As part of the transition to the NDIS, much of the focus of service delivery is to support consumers to achieve sustainable mental health care, via NDIS or other mainstream service provision.

Duties include:

  • Ensure the provision of high quality, recovery oriented care to people experiencing severe and persistent mental ill-health
  • Support clients in line with Mind’s Model of Recovery Oriented Practice
  • Deliver a mix of indirect and direct service provision, predominantly working alongside the client and their care team to ensure co-ordinated care
  • Assist with the transition into NDIS or other long term care options

Skills & experience

  • Tertiary qualifications (minimum certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind
  • Experience and expertise in working directly with people with mental health issues, complex needs, and with their families and carers
  • Demonstrated understanding of available community services, networks and supports
  • Awareness and understanding of the NDIS (desirable)
