The objective of the Kingston Aged Care Assessment Service (ACAS) is to provide a comprehensive, holistic assessment of an older person or a younger person with an age-related needs. The assessment is to identify the resources, strengths and problems of the person and to develop an individualised, coordinated support plan which focuses on client goals and re-ablement. ACAS assesses a person’s eligibility for Commonwealth funded care such as home care packages, restorative care, transition care, respite and residential care
The ACAS assessor position is designed to provide a comprehensive, holistic assessment of an older person or a younger person with an age related needs. All staff need to ensure the operational requirements of the program remain in line with the Commonwealth, the Victorian Department of Health and Human Services and Monash Health guidelines. This position is based at Kingston Centre, and entails travelling to assessments within the Cities of Kingston, Bayside and Greater Dandenong
