About Social Work Services

The Social Work team across Monash Health is responsible for the delivery of a broad range of Social Work services for clients across the lifespan in the inpatient and outpatient setting.

About the Role

The Social Work Department is looking for an experienced Grade 3 social worker to join and lead the social work team based at Monash Medical Centre (MMC). This is a full-time, 12 month fixed term maternity leave role from May 2023 until May 2024. This role involves leading a team of social workers and providing a clinical service in the Intensive Care Unit at MMC. You will supervise the team in the Chronic and Critical Care Stream. You will be working with patients from a diverse adult age group and offer support to patients and families around adjustment, crisis intervention and addressing complex social issues impacting on their recovery. This role is predominately based at MMC in Clayton but work across all sites may be required. Previous hospital social work and oncology experience is desirable.

This clinical and leadership role provides an opportunity to showcase your skills in working with people from a range of diverse backgrounds as well as completing complex psycho-social assessments and development of patient centred care plans. It is a requirement of this role to participate in the on-call rosters.

  • Deliver and drive Monash Health strategic intent to be relentless in our pursuit of excellence across Social Work and Allied Health.
  • Work closely with the Social Work Manager to provide streamlined services across all campuses and lead designated services/programs/portfolios.
  • Travel is required across Monash Health sites.

About You

You will be a dynamic, innovative and flexible clinical practitioner with a good understanding of the treatment and psycho-social issues facing individuals with chronic illness and their families. You will also have an understanding of the aged care sector, chronic health conditions and related services and referral pathways. You will be able to demonstrate skills in psycho-social assessments, problem solving, communication and hospital discharge planning. You ensure that high quality Social Work services that place the client at the centre of are provided. Education, quality improvement and evidence based practices will be your key drivers. Due to your proven clinical experience, you will bring knowledge of healthcare reforms and initiatives and challenges that public healthcare is facing now and in the future.

What you need

  • You must be eligible for AASW membership including continuing professional education
  • Proven experience in providing social work in acute and subacute settings.
  • Knowledge and experience working closely with inter-professional teams.
  • Current Driver’s Licence.
  • Current Working with Children’s Check
  • COVID-19 vaccinations
