The Support Coordination team connects participants with an NDIS plan to supports and services in New Horizons and the Community.

The team is the first point of contact for NDIS participants, families and Support Coordinators who are seeking information about New Horizons or who have chosen us to provide services for them under an NDIS plan.

Your role exists to coordinate the delivery of services to clients and supporting staff and clients in achieving the identified program outcomes.

We have a Full Time permanent opportunity supporting our customers in Sydney.

There is flexibility on your base location in Sydney as long as your meeting the expectations and needs of our customers. This includes travelling to engage with our customers and stakeholders where required.

Working days are Monday to Friday between 8am and 5pm with some flexibility required to support the needs of our customers and stakeholders.

The Day to Day:

• Meet or exceed Coordination of Support targets, and ensure that customer processes are carried out efficiently and within the required timeframes.
• Support a caseload of customers to increase their capacity to understand, monitor and manage supports of their choice
• Act as the first point of escalation for clients, sharing knowledge, information and expertise with other team members and stakeholders
• Increase customers’ engagement in the planning and delivery of their supports, promoting choice and providing exceptional experiences by ensuring that wellbeing is at the heart of everything we do.

Skills, Experience and Values:

• NDIS/Mental Health/Disability industry experience and knowledge
• Proven experience developing creative solutions to overcome day to day challenges for customers
• Proven experience working in a fast paced environment where you need to be adaptable, responsive and timely with stakeholder engagement
• Strong interpersonal and communication skills to build and maintain effective relationships.
• Experience providing and/or knowledge of Coordination of Supports, desirable
• Passion, Integrity, Respect

Equipment Requirements and Employment Check Criteria:

• Australian Federal Police Check from the last 12 months
• Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans
• NSW Working with Children Check (WWCC)
• NDIS Worker Screening Check
• Willingness to use personal vehicle for work purposes
