About the role

This position provides person centered care to consumers through a wide range of clinical services to consumers with multifaceted and complex clinical and social care needs. Ensuring consumers maintain choice and control of their care is critical to this role, balanced by the ability to undertake thorough clinical assessments and liaise with a multi-disciplinary team to ensure consumers’ safety and wellbeing is the priority. Key Responsibilities of the role

  • Work collaboratively with the Home Care Manager and Case Management team to assess Abound Communities capacity to onboard and ongoing service provision of a consumer during the enquiry process.
  • Completion of the required clinical assessments upon consumer intake,
  • Supporting Case Managers with Clinical Care for Home Care Package consumers;
  • Provide clinical reports and follow-up to the case manager and escalate to Home Care Services Manager and Director of Nursing as needed.
  • Coordinate consumer care needs, including assessment and developing individualised Care Plans
  • Accept HCP referrals via My Aged Care Portal and enter relevant data into VisualCare. This may include, but not limited to; Progress Notes, Assessment Outcomes, Consumer Goals, Community and Clinical referral details, recommendations and observations pertaining to consumers and their care needs.
  • Engagement with consumers, their families, and/or their representatives to link with local networks, GP's, allied health, hospital teams and clinical specialists to assist consumers with the management of their assessed care needs.
  • Provide leadership and support to Support Workers to ensure the consumers care needs are met.
  • Liaise with consumers, their families and carers, and members of a multidisciplinary team to achieve positive consumer outcomes.
  • Ensuring Compliance; clinical oversight of care plans and assessments, provide regular updates and reports to the Home Care Manager and the Director of Nursing regarding both on-going monitoring concerns, intake of new consumers, outcomes of clinical assessments and reviews, participation in home care meetings, mandatory training ect.
  • Regularly monitoring, maintaining and reviewing consumer’s care needs and care plans in conjunction with the case manager, to ensure that the consumers assessed care needs are addressed.
  • Deliver all services with a strong focus on providing the consumer with an exceptional customer service experience, maintaining the Code of Conduct for Aged Care and behaviour in a manner that displays Abound Communities Values.
  • Build and maintain strong relationships with team members and external stakeholders.
  • Adhere to Abound Communities Policies and Procedures.
  • Maintain compliance with the Aged Care Quality and Safety Standards
  • Other duties and tasks as required

To be successful in this role you will have:

Essential:

  • Registered as a Nurse with Australian Health Practitioner Regulation Agency (AHPRA) and holds a current practicing certificate.
  • Experience in completing and facilitating Communication, Vision, and Hearing Assessments.
  • Experience in Personal Hygiene, Skin Integrity, Dietary, Hydration and Oral Care Assessments.
  • Experience in completing Continence Assessments (Urinary and Fecal) and development of a Consumer Management Plan.
  • Demonstrated case management experience and/or knowledge of the Community/Aged Care system and home care packages.
  • First Aid Certificate
  • Valid Driver’s licence and a reliable, registered vehicle with comprehensive car insurance
  • Current National Police Check
  • Covid 19 vaccination certificate including the booster shot
  • Evidence of right to work in Australia
