To continue operating at the highest standard we are seeking a Social Worker to join the team. In this role you will support our team in the delivery of services funded by the Department of Communities & Justice (DCJ).
You will report to the Head of Organisation, and will have interactions with a wide range of clients from diverse backgrounds. As part of the functioning of the role, the successful applicant would be comfortable providing case management services independently to clients ranging from low to high-complexity, and deliver, or assist in the delivery of group programs.

Objectives of the Role
In accordance with the ongoing functioning of Phoenix House Youth Service the position will be required to:
- Manage the relationship between Phoenix House and the DCJ, specifically in-relation to the delivery of services under the contract, data reporting mechanisms, and relationship management.
- Manage a formal caseload of up to 9 clients per month, with
- A client turn-over of 5 – 6 per month.
- Deliver, and assist in the delivery of group programs with clients.
- Assist in the development, and management of programs outside of the annual Business Plan to provide supplementary assistance to groups of clients, acquit new / existing grants, or develop areas identified in the organisational strategy.
Key Responsibilities
As a Social Worker, the role will:
- Meet with clients one-to-one to provide an assessment of their needs, referral support and formal advocacy.
- Liaise with other Phoenix House staff members to coordinate other wellbeing supplements,
- Liaise with external stakeholders directly related to advocacy and assistance services for clients (including, but limited to education, juvenile justice, local councils, housing providers, other social service providers, etc.).
- Maintain established timelines in-relation to client service periods,
- Assist in maintaining the delivery of group service programs,
- Assist in the collaborative growth of Phoenix House programs,
- Provide other assistance where necessary in the delivery of services under the acquittal of government-funded contracts.
Role Capability
The role is expected to:
- Implement transparent and accountable decision making in relation to client needs,
- Maintain a high level of communication and collaboration with staff and others;
- Provide competent and effective care to vulnerable youth on Sydney’s North Shore,
- Maintain a strong oversight and understanding of the DCJ’s contractual requirements for social service.
Criteria
The criteria for the Social Worker role includes:
Essential:
- Minimum 2 years' experience in the delivery of case management services,
- Experience working with young people / mental health & homelessness.
- Associate Diploma, Advanced Certificate, or higher equivalent tertiary qualification in a related field.
- Comfort in using data reporting systems (including a CRM [Customer-Relationship-Management] system).
- Criminal History Record Check and Working with Children Check required.
Desirable:
- Experience in the delivery, planning and / or process improvement of group programs.
- Experience in the areas of education, special-assistance education or study-support programs.
- Strong relationship management skills.
Duties
Case Management (60%)
- Provide independent case management services for clients (low to high-complexity) experiencing, or at-risk of homelessness, or as part of targeted early-intervention programs.
- Provide case management services outside of client interaction, including but not limited to: advocacy services, stakeholder liaison, housing or service provider referrals, etc.
- Provide assistance with, or independently engage in stakeholder engagement activities related to the promotion of Phoenix House’s case management services.
Administration (20%)
- Continual, relevant and accurate data-entry into the internal data mechanisms of Phoenix House, and data-entry into the external data mechanisms of Phoenix House stakeholders regarding client services for both individual and group delivery.
- Assistance with administrative tasks related to the ongoing operations of Phoenix House, as-needed.
Group Activities & Public Engagement (20%)
- Assisting in the development, and continued delivery of group programs for clients.
- Organisation of, and / or participation in public engagement activities.
