Join our team as an SRS Clinician, where you'll play a vital role in enhancing the wellbeing of residents in local Supported Residential Services (SRS). You will conduct comprehensive, client-centred assessments to identify care goals, address unmet health and social needs, and manage complex care situations to prevent instability and evictions. Collaborating with the Homelessness and Health Outreach Service (HHOS), you'll help connect residents with long-term housing solutions and provide essential support services to improve health outcomes and foster community connections. This position is based at Mornington Community Health centre, but may require work across all Peninsula Health sites.

*Please refer to the salary circular for Allied Health Professionals Enterprise Agreement (Vic Public Health) for salary range. Sub-grading will be based on years of experience.
