the opportunity

PRONIA is seeking a suitable applicant for the Case Manager Position on a full-time basis located initially at Brunswick and then moving to Oakleigh, may also be required to travel to other sites across Victoria. Reporting to the HCP Coordinator, you will be responsible for providing support to our Home Care Package clients, monitor care recipient needs, seek opportunities for growth in the Home Care Packages program and undertake additional tasks in line with the overall position relating to PRONIA.

Role requirements

The successful applicant will have, amongst other attributes, appropriate tertiary qualifications preferably in Social Work, Nursing or Allied Health fields, minimum 1-year experience in community aged care or case management, awareness of issues for Greek speaking carers and the ability to work cooperatively and autonomously within a team environment.

Proficiency in verbal and written Greek and English, coupled with excellent time management skills is essential. Proficient use of Microsoft Office such as Word, Excel, Access, Internet and E-mail are also required.

Competitive remuneration according to qualifications & experience under the Social, Community, Home Care & Disability Industry Award 2010. Salary packaging available.
