NDIS Support Coordinator / Psychosocial Recovery Coach

Come join our fun and supportive team at Quality Care Victoria in the mixed role of a Support Coordinator and Psychosocial Recovery Coach!

As a Support Coordinator and Psychosocial Recovery Coach, you will:

  • Coordinate NDIS and mainstream services for NDIS participants.
  • Work alongside participants on their recovery journey focusing on their strengths and having an optimistic perspective.
  • Building capacity and helping participants better understand their NDIS plan.
  • Providing coaching and recovery planning or support coordination.
  • Empower participants to make their own choices and achieve their NDIS goals.
  • Assisting participants to source and connect with services.
  • Liaising and coordinating with external agencies.
  • Work as a part of larger care teams and coordinate communication with other key staff such as support workers and therapist.
  • Report writing and case noting in a timely manner.

Some requirements we have are:

  • A Victorian Drivers' license and car
  • Positive attitude with a learner's mindset
  • Committed to work flexible work hours across the week - Mon-Friday role.
  • Confident in communicating with a variety of people and conducting in home visits.
  • Ability to use basic technology and software on a mobile and desktop
  • Experience with Google Suites
  • A current First Aid and CPR Certificate
  • Work from office position with in home visits and some flexibility.
  • Complete COVID 19 vaccination
  • Relevant qualification and/or prior experience equivalent.

  • A strong understanding of the needs of people living with psycho-social disability.
  • A person centred and strengths-based approach
  • The ability to think innovatively and creatively to help clients achieve their NDIS plan goals
  • Case management and engagement skills
  • The ability to collaborate and negotiate with multiple stakeholders carers
  • Excellent time management, budgeting and administrative skills
  • Bilingual skills highly considered!
  • Transferrable skills highly considered!
