The Role
A Support Coordinator is responsible for supporting the National Disability Insurance Scheme (NDIS) Participants to activate, interpret and implement their NDIS Plans. As a Support Coordinator, you will work with scheme participants between the ages of 0 – 65 with varying complexity levels. You will coordinate service providers and supports, liaise with government agencies, obtain quotes for services, organise service agreements and monitor Participants plans. You will also provide regular Support Coordination reports to the NDIA for each Participant you support.

Qualifications & experience
- Minimum Certificate IV in Disability or related field
- Demonstrated knowledge and experience of NDIS
- Current First Aid and CPR Certificate (or willing to obtain)
- Victorian Driver's license and vehicle
- NDIS Worker Screening check (or willing to obtain)
- Working with Children Check
Tasks & responsibilities
- Liaising and building rapport with key stakeholders such as: NDIS Providers and Agents, community venues, community partners, along with other revenue streams or allied health care teams.
- Monitor plan outcomes and expenditure to ensure the services remain within the plan budget for participants.
- Independently manage a varied caseload of participants in the community ensuring quality of care and meeting client objectives and requirements.
- Conducting initial assessments
- Researching and referring participants to relevant services when required with a focus on Recovery.
- Ensure participants are working towards achieving their goals and being able to clearly document progress in reports
