Based in Frenchs Forest, CCNB is one of the largest community-led organisations in the Northern Sydney Region, providing services to older people, those with dementia, people with disabilities, severe and persistent mental illness as well as those with complex health needs.

Our focus is to support people, their families and carers to navigate the health and social care systems to access the services they require, when they require them.

The Role

The Care Navigator is the first point of contact for new and existing clients contacting CCNB. The Care Navigator is responsible for:

  • Providing exceptional customer service to clients by providing information on services, impartial information, advice and guidance.
  • Working with clients to ensure they can access and engage services that will meet their needs. Services may be provided by CCNB or available in their local community.

CCNB prides itself on the importance of the Care Navigator Team in supporting clients in the first steps of their journey and linking them into their next stage of care and support.

About You

Ideal candidates will:

  • Have a passion for delivering excellence in customer service
  • Enjoy working in a dynamic and fast-paced environment and have an enthusiastic and resilient approach
  • Relevant experience in delivering information, advice and guidance in a health, community, wellbeing or similar sector.
  • Highly developed interpersonal skills including consultation, engaging customers, openness, fairness, flexibility, liaison and problem-solving.
  • Excellent skills in communication (verbal/written), administration & report writing.
  • High level of computer literacy and experience using databases.
  • Knowledge of, or willingness to learn about services and systems in the areas of Aged Care, Disability (NDIS) and Mental Health.
