Flourish Australia has an exciting new opportunity for a Support Coordinator to join our team providing support coordination services in Broken Hill!

The position available is:

• Part-time (60.8 hours per fortnight)
• Working Monday to Friday (days to be negotiated)
• Working in our established Broken Hill centre
• Level 4 on the Flourish Australia Enterprise Agreement (2018) equivalent to $77,000 per annum + 10.5% Super

About the Opportunity

Support Coordination aims to support people with a permanent and significant disability that affects their ability to take part in everyday activities. As a Support Coordinator, you will establish a positive collaborative relationship with the person and their support network. You will assist the person to identify, link with, and coordinate supports in their local communities, to build skills, overcome barriers and achieve goals.

In this role, you will give people more choice and control over how, when and where those supports are provided. You will be fundamental in the implementation of the person's plans and the achievement of their goals. In this position, you will also be required to ensure that people who have expressed an interest in accessing the NDIS and/or receiving support from Flourish Australia are engaged and supported to access suitable services.

Selection Criteria

• A tertiary qualification in the behavioural sciences or a health-related discipline
• Understanding of the challenges faced by people with a lived experience of a mental health issue.
• Demonstrated experience in the provision of support and coordination for people with a lived experience of a mental health issue.
• Current Australian Driver's Licence.
