Palliative Care South East (PCSE) provides palliative care in the south east metropolitan region of Melbourne. We support people with a life-limiting illness to live well and, in doing so, provide support at home to improve the quality of life of individuals, carers and families. Our specialist palliative care services are provided to people of all ages, free of charge, in the Local Government areas of Cardinia, Casey, Greater Dandenong and part of Kingston.
As a Palliative Care Social Worker, you'll work closely with the multidisciplinary team to provide comprehensive support to individuals and families. Your role involves taking a holistic approach to client and family work, with expertise in the psychological, social, and emotional dimensions of end-of-life.
Your responsibilities include conducting episodic casework based on internal referrals, leveraging your expertise to assess social determinants of health, and delivering psychosocial interventions. You'll develop tailored care plans to enhance the quality of life for clients and families, assisting them in navigating healthcare systems and accessing necessary resources.
This is a 12-month fixed-term contract (maternity leave cover), requiring a commitment of 0.9 FTE, starting in March 2024.
