St John of God Health Care

St John of God Social Outreach Casa Venegas supports adults with Mental Health issues who are experiencing Homelessness to transition to independent living through a recovery-based framework. Services provided include accommodation, advocacy and liaising with mental health services on behalf of the consumer.

Opportunities for Community Support Workers have become available within our passionate and dedicated team.

Based in Burwood, these positions are responsible for a high standard of care and quality service to the consumers of St John of God Social Outreach (SO) Casa Venegas by working with the Support Team to design, develop and implement case management strategies and individual support plans for the consumers.

Positions may be either as permanent ongoing or on a casual contract basis.

The successful candidate will have:

  • A willingness and capacity to embrace the Mission and Values of St John of God Health Care.
  • Minimum qualification of Certificate IV in Community Services, Mental health Peer worker or equivalent.
  • Experience working in a community, mental health or social care services role and the implementation of individual support plans for consumers.
  • Understanding of the principles of Recovery orientated mental health practice and strong case management skills.
  • Demonstrated ability to prepare written and verbal reports on consumer outcomes and performance against KPIs.
  • Excellent interpersonal skills including conflict management.
  • Ability to work as part of a united team to deliver specific outcomes to consumers.
  • Ability to establish and develop community partnerships, engaging and linking residents in appropriate community activities or events.
  • First Aid Level 2 and CPR certificates.
  • Possession of a valid NSW Driver’s license.
