The opportunity

We are currently looking for an experienced community Occupational Therapist to support our clients throughout the Bunbury / Bussleton region. In this role, you will be responsible for the delivery and coordination of professional Occupational Therapy services across a number of programs (CHSP / Reablement / HCP) to achieve improved occupational, social and health outcomes, optimum independence and improved quality of life for our clients. Working in a community aged care setting, you will work independently completing home-based assessments and provide appropriate recommendations, intervention and evaluation. Experience in care management and transdisciplinary practice would be an advantage. This role will be based out of our Bunbury / Busselton office however due to the nature of the service you will spend time traveling to deliver care to our clients in their own homes. A pool car, the technology required to do your role, and orientation to and support in the role will be provided. Regular PD in-house and peer supervision is available.  

About you

To be considered for this role, you will meet the following criteria: 

• Tertiary qualifications in Occupational Therapy
• Current registration with Australian Health Practitioner Regulation Agency (AHPRA)
• Previous experience working as an OT in a community setting
• Ability to identify and provide appropriate advice on complex care needs by exercising initiative and innovation in the application of professional practices under limited direction.
• Previous experience in minor home modifications and equipment prescription
• Ability to use a reablement/restorative model of care to enhance the independence of our clients
• Excellent interpersonal and communication skills (written and verbal)
• High level computer skills
• Ability to work autonomously as well as work well within a multi-disciplinary team
• Highly organised, strong attention to detail and ability to meet deadlines
• Valid Australian drivers licence 

Key responsibilities

• Community based role with a strong focus on enhancing the independence of our clients
• Plan and carry out required services to clients following guidelines for assessment and treatment required
• Maintain a close liaison with other community resources to ensure the ongoing care
• Liaise with families/care providers by providing ongoing support, counselling and education
• The ability to work independently
• Plan and carry out relevant training to staff
• Prepare reports as required

Under the Aged Care act 1997, successful candidates will be required to provide a satisfactory National Police Clearance prior to commencement.
