Silver Chain Community Palliative Service is looking for an experienced Social Worker to join the Seven Hills Palliative Care team on a Permanent Part Time basis working a minimum of 42 hours per fortnight. This role is based out of Seven Hills service centre, between the hours 8.30am - 5.00pm with some flexibility in start and finish times.
You will work closely with clients, their families and relevant others to take a holistic approach to the social and health needs of clients with an ultimate goal of reducing the number of unplanned admissions to hospitals by successfully integrating the care needs of each client.
