Role overview:
- Provide psychosocial support for children and families requiring more intensive support. This may include supports to increase a child and/or family’s capacity for social and community connection, independent daily living, financial management, physical health and wellbeing etc.
- Develop and review Individualised Support Plans in collaboration with the child, family and other professionals as needed, to identify and plan the required psychosocial supports that are need for the child and family.
- Provide advocacy or support self-advocacy for carers where appropriate.
- Provide assistance, information and referrals for alternative services as appropriate.
- Liaise with and develop collaborative working relationships with key service providers in order to ensure ease of access to other services for children and families as required.
- Provide support in diverse settings, including outreach and home visitations as required.
- Provide support in diverse modalities, including individual, family-based, and group-based formats as appropriate.
- Relate to children and families in a manner which is relevant and appropriate to their developmental and cognitive level of functioning and provide services in a professional and friendly manner.
- Maintain appropriate documentation regarding intervention and support provided.
Requirements:
- Minimum of a Certificate IV in mental health or community services related discipline (ideally a completed Degree).
- Previous experience in a Support Worker, or related, role.
- In-depth understanding of issues pertaining to mental illness/or disabilities and their impact on children, families and communities.
- The ability to work autonomously and as part of a team.
- Demonstrated effective listening, empathy and communication skills.
- Excellent negotiation, problem solving and decision making skills, as well as the ability to work to time frames, meet deadlines, and work in challenging situations.
- Demonstrated ability to work with children, families, carers, government agencies and community services.
- Competence with Microsoft Office.
- Must be eligible to apply/currently hold a Blue Card.
- Must hold/be willing to apply for NDIS worker screening clearance.
- Current Driver's license.
