Within The Salvation Army Social Mission programs, we are involved in a broad integrated range of services with an aim to bring support to the some of the most vulnerable and disadvantaged adults experiencing homelessness in Melbourne's Central, Northern and Western metropolitan areas.

The Salvation Army Australia's Homelessness Stream provides high quality, person centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.

THE ROLE

We are looking for a talented and passionate case manager to work in our Melbourne Streets to Home (MS2H) program.

MS2H offers holistic and intensive case management support. Support domains include:

  • Housing and homelessness
  • Mental health
  • Physical health
  • AOD
  • Family violence
  • Legal
  • Financial
  • Employment
  • Education
  • Social, family and community connection
  • Anything else important to ending the client's homelessness/ improving their health

MS2H is distinctively different to other supportive housing and assertive outreach programs on offer. The MS2H service is led by Launch Housing and delivered in partnership with The Salvation Army and Bolton Clarke to provide a multi-disciplinary team approach. MS2H works with the highest acuity clients directly referred through the Rough Sleepers Initiative (RSI) and works from a Housing First model. The MS2H cohort have high morbidity presenting with poor physical health including physical and intellectual disabilities and premature ageing.

Case management in MS2H is not a linear process and is done at the client's pace. As many of the MS2H clients have experienced systemic discrimination and trauma, the relationship between Worker and client is pivotal to the case management process. Workers hold their clients in unconditional positive regard and seek to empower their clients by fully involving them in the case management process and being as transparent as possible. Workers seek to be strengths based, and use a power with, rather than a power over approach.

The position will be based in West Melbourne, though may be required to work from other TSA sites, such as North Melbourne and Kensington.

HOURS AND SALARY

This is a permanent full time position, 38 hours per week.

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, level 5.

  • To deliver high quality support services to vulnerable people
  • Knowledge of homelessness, Housing First approach, crisis intervention and the ability to collaborate with other program areas
  • Demonstrated skills in engaging & working with people with complex support needs including AOD, mental health, out of home care and violence
  • Assessment skills and knowledge of a range of theories including trauma-informed practice and strengths-based approaches
  • Demonstrated capacity to time manage within a crisis setting whiles maintaining required documentation and IT systems to DFFH and organisational standards
  • Competent to work solo when required and respond effectively to critical incidents in a safe manner
