Access Health operates within The Salvation Army's Alcohol and Drug Services stream, Australia. Access Health provides accessible, responsive Primary Health Care that enhances the health and well-being for people who inject drugs, street sex work and/or experience homelessness. Access Health offers non-appointment based services that are free of cost and facilitates linkages to mainstream services. The program works in partnership with other health and support services to provide clinical and non-clinical health services and practical support.
We have an exciting opportunity for a Social Worker to be the fist point of contact with people attending the Access Health Services in St Kilda.
You will play a central function to the Access Health daily activities including: engaging service users, providing orientation, assessment, triage, information, referral and advocacy to people attending the service, managing the flow of service users through the program and coordinating activities and information to other Access Health professionals.
This is a permanent part time position, 3 days per week (22.8 hours). Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services Award Level 5.
