About The Way Forward:

The Way Forward Disability Consulting is a registered National Disability Insurance Scheme (NDIS) provider based in Brisbane, Queensland. The Way Forward provides innovative Capacity Building Supports for participants of the NDIS. We work within a person-centred framework, utilising principles of social justice and Right Relationship, to help participants achieve their goals, wishes and desires and to get the best out of their NDIS plan. As an independent provider of supports, The Way Forward are not affiliated with any service providers and can help participants to exercise true choice and control.

We work across Brisbane, The Redlands, and parts of Moreton Bay, and provide supports in participant’s home or location of their choosing. The Way Forward specialises in supporting NDIS participants who have complex medical conditions, participants who require assistance with hospital discharge and housing supports (SIL and/or SDA) and participants who reside in residential aged care (YPIRAC).

The Position:

The Support Coordinator provides support coordination services to participants of the NDIS to assist them in implementing the supports outlined in their NDIS plan. This includes informal, mainstream and community supports as well as funded supports.

Working with the National Disability Insurance Agency (NDIA) and multiple stakeholders, the Support Coordinator assists participants to build their capacity and empowers them to utilise their plans and achieve their goals.

The position is available as follows:

  • Part time, Full time, or contract; negotiable for the right person depending on how you want to work (22.5hrs/week minimum).

As a Support Coordinator, your duties and responsibilities will include:

  • Coordinate, monitor and report on supports in place in participants NDIS plans.
  • Assist participants to identify supports required to meet their needs.
  • Use your local expertise to assist people connect to their communities.
  • Assist participants to build their capacity to navigate the NDIS and implement their own supports.
  • Work with participants to develop and set goals, engage with providers, and prepare for reviews.
  • Be the key contact for the participant for service challenges, complaints, major changes, and plan reviews.
  • Mentoring and sharing your skills and knowledge with other staff.

About You:

To be successful in this role, it is important you meet the following key selection criteria:

  • Tertiary qualifications and / or extensive experience working with people with disabilities, and people from diverse cultures and backgrounds.
  • Experience in Support Coordination, Service Coordination, Case Management or as an LAC/Planner.
  • Highly developed written and verbal communication skills.
  • Proficiency in the use of Outlook, Word, and electronic CRM databases
  • Experience meeting relevant legislation including upholding privacy and confidentiality.
  • Organisational skills to effectively manage your own workload (this is a work from home position), meet deadlines and key performance indicators.
  • Understanding of the principles of inclusion, strengths based practice and the social model of disability.
  • Positive, friendly personality who enjoys working in this sector and is keen to be part of building a great culture from the ground up.
  • Current Positive Notice – working with children screening check (Blue Card)
  • Current Yellow Card Exemption/NDIS Worker Screening check (or willingness to obtain prior to commencement)
  • Current driver’s license and access to a reliable, registered, comprehensively insured vehicle. You will at times be required to travel to participant, stakeholder, and team meetings.
