Your Community Health provides a comprehensive range of medical, dental and allied health services and support programs to anyone, through a combination of outreach, client-based and centre-based activities. Our purpose is to enable health, wellbeing and dignity for all people in northern Melbourne by providing responsive and accessible services. We provide priority access to those most at risk of health inequalities and those that may face barriers to accessing traditional health services.

About the role

Your Community Health is seeking an enthusiastic and experienced Social Support Worker to join the Social Support Team for this casual position. There may be future opportunity for ongoing, part time work.

Reporting to Social Support and Men's Shed Coordinator the Social Support Worker position will enhance participants' well-being, support their independence and meet their cultural, physical, intellectual, physiological, social and emotional needs.

Your passion and commitment will see you:

  • Ensure that the provision of services and supports are safe, evidence informed. and meet the ACQS aged and NDIS disability quality standards.
  • Provide personal care for clients as required within your scope of practice.
  • Maintain client documentation and follow the record keeping policies of Your Community Health. This includes the use of the Client Information Management System (CIMS) - TrakCare, entering daily consumer notes, maintaining daily attendance records, and entering daily statistics.
  • Ensure daily collection and processing of client's fees using online CIMS.
  • Complete, review and annually update Goal Directed Care Plans in partnership with participants, as detailed in the Social Support Program Practice manual

To succeed in this role you will have:

  • Certificate III in Aged Care/Individual Support and/or Cert IV in Lifestyle & Leisure.
  • Qualifications in disability support highly desirable.
  • Experience working with older people and people with disabilities living in the community, including multiple sclerosis and dementia highly desirable.
  • Hold a current Victorian Drivers Licence

Requirements:

The successful candidate's offer of employment will be subject to their ability to provide:

  • a current Working with Children Check (employee only)
  • Police Check with no disclosable outcomes that is no older than 3 months. (Issued by employer at no additional cost to the candidate)
  • Proof of 2 Covid-19 Immunisations and a Booster (3rd dose)
  • NDIS registration
