About the role

A rewarding opportunity to provide a range of services to children, young people, their family and carers, who are engaged with Child Protection, to support them as they exit residential care and also prevent them from entering residential care.

The role will be responsible for providing intensive case management and support to children, young people, their families, and carers subject to a Targeted Care Package (TCP).

The role will involve developing, implementing, and reviewing care plans, collaborating with care teams, engaging with clients, their families, and carers to ensure the provision of necessary supports and interventions are implemented to promote stability and positive outcomes.

This role is a 12-month fixed term contract (Parental Leave Replacement)

This role will be based in Tullamarine and will be required to travel across Melbourne, predominately in the North and Western suburbs to meet with clients and their families.

Key responsibilities include

  • Provide direct support to children and young people, their family and carers for the duration of the package (TCP).
  • Provide risk assessments and case management to support the child or young person’s case plan.
  • Coordinate care team processes for the child or young person in partnership with other services to develop, deliver and review the package plan.
  • Ensure all Aboriginal children and young people are provided with appropriate cultural supports in accordance with their Cultural Support Plan.
  • Advise Child Protection and TCP staff of any changes, issues, or risks to the child or young person’s living arrangements or package delivery.
  • Ensure CRIS/SP is up to date with activity records, service partner consultations, decisions, and actions for the duration of the TCP (includes post statutory involvement).

About you

  • Understand the issues relevant to the placement of children in alternative placement contexts and with child protection involvement.
  • Ability to contribute to the development, implementation, monitoring, and evaluation of case plans.
  • Experience in case management within the child and youth sector where multiple service providers may be involved.
  • Knowledge of the NDIS and experience working with children and young people who may have disabilities and/or mental health concerns.
  • Knowledge of therapeutic frameworks, care planning and the ability to provide trauma informed care.
  • Able to identify children and young people who may be at risk and can understand and deliver mandatory reporting obligations.
  • Knowledge of external services that can provide support to children and young people, their families and carers, with the ability to refer people to those services.

To be considered for this position, you must have

  • A minimum of a Certificate IV or greater level in Child, Youth & Family Intervention, or an equivalent qualification, in addition to the completion of the mandatory units of competency for residential care workers.
  • A relevant tertiary qualification in Social Work, Psychology, or equivalent will be highly regarded.
  • Victorian Drivers’ Licence.
  • Employee Working with Children Check; (or willingness to obtain).
  • NDIS Worker Screening check (or willingness to obtain).
  • Right to work in Australia.
  • Current First Aid and CPR (or willingness to obtain).
  • Ability to safely transport clients. Use of personal vehicle may be required (km’s reimbursed).
  • Comprehensive Car Insurance.
