We are looking for a dedicated, compassionate, and proactive Care Coordinator to join our team and help us make a meaningful difference in the lives of those we serve.

Key Responsibilities:

In this role, you will be responsible for:

  • Contributing to the Development, implementation, monitoring and review Behaviour Support Plans (BSPs) and care plans to align with participants' NDIS goals and promote their overall quality of life.
  • Collaborating with NDIS Support Coordinators and building strong relationships with key stakeholders to ensure seamless service delivery.
  • Preparing regular progress reports (weekly, fortnightly or monthly) on participant needs, outcomes, and achievements.
  • Overseeing the delivery of high-quality, personalised care, ensuring that participant needs are met with professionalism and empathy.
  • Ensuring compliance with health, safety, and regulatory standards for both participants and staff.
  • Managing activities such as personal care and medication administration to meet required standards.
  • Participating in a monthly on-call roster and providing after-hours support to staff when needed.
  • Reporting any reportable incidents in accordance with NDIS Quality and Safeguards requirements and ensuring proper documentation and follow-up actions are taken.

Experience & Skills:

We are looking for candidates who possess:

  • At least 2 years of experience working as a Care Coordinator or in a similar role within the disability or mental health sector.
  • Strong knowledge of the NDIS, including mandatory reporting, Quality and Safeguards, NDIS Price Guide, and budget flexibility.
  • Ensuring that the staff deliver high-quality and personalised care to participants.
  • Ensuring health and safety compliance for participants and staff
  • Experience managing multiple, complex cases concurrently, with attention to participant life goals.
  • Exceptional attention to detail, problem-solving abilities, and strong organisational and planning skills.
  • Ability to work independently with minimal supervision and support a diverse range of participants with varying needs.
  • Proficiency in using computer systems, including Client Management software and PRODA.
  • Familiarity with NDIS reporting requirements and the ability to accurately document and report incidents when required.
